Out-Of-Network
For other insurance plans I am able to offer a Superbill for those seeking Out-Of-Network (OON) reimbursement.
If you have OON insurance benefits, you can use a service called Mentaya, that will file claims and handle the paperwork to make sure you get reimbursed. They charge a 5% fee per claim. If you would like Mentaya to help you with reimbursement click the button below.
